By checking this box, you understand that if you are NOT already waiver-approved, the process to review and approve a new application could take up to three (3) months or longer, and that you would like MealsUSA to help you apply.

If you are not yet waiver-approved, the process to obtain our services may take up to three months or more. Throughout this period, you and your physician will need to fill out several forms. You will also undergo three interviews to confirm your eligibility, and you must submit bank statements for the past five years to meet income verification requirements. If these statements are unavailable, we will assist you in acquiring suitable documentation.
